OA has a dedicated referring provider line. To refer a patient to us, please call 937.439.1981. This is our dedicated physician line to get your patient in as soon as possible. We guarantee to see patients that are referred to us in less than 24 hours. You may also send electronic/fax referrals and we will call your patients to schedule an appointment.
